HIPAA NOTICE OF PRIVACY PRACTICES

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.  PLEASE REVIEW IT CAREFULLY.

MY PLEDGE REGARDING PROTECTED HEALTH INFORMATION (PHI)

In this Notice, I use the terms "I," "ME," and "MY" to describe Diana Price, MS, RDN, owner of Honest Nutrition USA, and/or anyone employed by Honest Nutrition USA.  This may include other clinicians and staff members.

 

Protected health information (“PHI”) means any information, whether oral or recorded in any form, that is created or received by me and relates to your past, present, or future physical or mental health, health care treatment, and payment for health care services.  PHI includes demographic information such as your name, social security number, address, and date of birth and either identifies you as an individual or there is a reasonable basis to believe that the information could be used to identify you.

 

I understand that the information about you and your health care is personal and I am committed to protecting your PHI.  As part of your treatment, I create a record of the care and services you receive from me.  I need this record to provide you with quality care and to comply with certain legal requirements.  

 

This Notice applies to all of the records of your care generated by this practice.  This Notice will tell you about how I may use and disclose your PHI.  This Notice also describes your rights to your PHI and describes the obligations I have regarding the use and disclosure of your PHI.  

 

I am required by law to:

  • Ensure that your PHI remains private and confidential;  

  • Notify you about how we protect PHI about you;

  • Explain how, when, and why we use and disclose PHI; and

  • Follow the terms of the Notice that is currently in effect.

 

We reserve the right to change the terms of this Notice and to make new Notice provisions effective for all PHI.

 

USES AND DISCLOSURES OF PHI

The following categories describe different ways that we may use and disclose PHI without your written authorization.

 

For Treatment: We will use and disclose your PHI to provide, coordinate, or manage your healthcare and any related services. This includes the coordination or management of your healthcare with a third party.

 

Payment:  Your PHI will be used, as-­needed, to obtain payment for your healthcare services. If you pay out of pocket and in full for a healthcare service, then you have the right to restrict certain disclosures of your PHI to your health insurance.

 

Healthcare Operations: We may use or disclose, as-­needed, your PHI to support the business activities of Honest Nutrition USA. These activities include, but are not limited to, quality assessment activities, employee review activities, training, licensing, and conducting or arranging for other business activities.  For example, we may use PHI to review our treatment and services or to evaluate the performance of the dietitian who is providing your services. We may also combine PHI about many Honest Nutrition USA patients to decide what additional services Honest Nutrition USA should offer, what services are not needed, and whether certain treatments are effective. We may also disclose information to other Honest Nutrition USA personnel for review and learning purposes.

 

Other Permitted and Required Uses and Disclosures:  will be made only with your consent, Authorization, or Opportunity to Object unless required by law. You may revoke this authorization, at any time.

 

YOUR RIGHTS 

Following is a statement of your rights concerning your PHI.

 

You have the right to inspect and copy your PHI.

 

You have the right to request a restriction of your PHI.  This means you may ask us not to use or disclose any part of your PHI for treatment, payment, or healthcare operations. You may also request that any part of your PHI not be disclosed to family members or friends who may be in your care or for notification purposes as described in this Notice of Privacy Practices. Your request must state the specific restriction requested and to whom you want the restriction to apply.

 

You have the right to request to receive confidential communication from us by alternative means or at an alternative location.  You have the right to obtain a paper copy of this Notice from us.  Upon request, even if you have agreed to accept this Notice alternatively i.e. electronically.

 

You may have the right to amend your PHI. 

 

You have the right to receive an accounting of certain disclosures we have made, if any, of your PHI. We reserve the right to change the terms of this Notice. Any changes to this Notice will be posted on our website (www.honestnutritionusa.com) within 30 days of making any changes.
